This program allows participants to become certified for a period of three (3) years and to complete "No Change Affidavits" annually.

CONSTRUCTION INDUSTRY APPLICANTS
Construction Industry Personal Net Worth Statement

For Applicants in the Construction Industry applying for Cook County Certification or submitting a No Change Affidavit as MBE and/or WBE, each qualifying owner of the Applicant firm must complete the "Personal Net Worth" form and all non-qualifying owners who possess 20% or more interest in the applicant firm are required to complete this form and submit with certification documents.
