Cook County Office of Contract Compliance certifies Minority and Women Businesses Enterprises (MBE/WBE) for the County to participate in the County's Affirmative Action Procurement Program. This program allows participants to become certified for a period of three (3) years and to complete "No Change Affidavits" annually.

For Applicants in the Construction Industry applying for Cook County Certification or submitting a No Change Affidavit as MBE and/or WBE, each qualifying owner of the Applicant firm must complete the "Personal Net Worth" form and all non-qualifying owners who possess 20% or more interest in the applicant firm are required to complete this form and submit with certification documents.
