The Cook County Department of Public Health (CCDPH) is committed to protecting and promoting health and preventing illness, disability and premature death among all the residents of suburban Cook County. 

Services include immunizations; STD testing, treatment and counseling; family planning; dental clinics and a variety of case management services for infants, pregnant women and families. Through collaborative efforts with private medical providers, Federally Qualified Health Centers (FQHCs), faith based organizations and community agencies; the Department links residents to needed medical and social services within the area, such as mammography and cervical cancer screenings. 

Other services provided by CCDPH include enrollment opportunities for Access to Care, a county subsidized program providing low cost medical services to qualified suburban Cook County residents, as well as state-funded insurance programs. 

The Department protects Cook County communities from potential environmental health hazards and communicable disease outbreaks through surveillance and inspections.

CCDPH emphasizes preventive approaches to disease and injury through innovative and educational activities such as lead poisoning prevention and tobacco cessation programs. The Community Preparedness and Coordination unit is committed to protecting residents from large scale disease outbreaks or natural disasters through preparation efforts with first responders, municipalities, schools and businesses.

Discount Card

In September 2007, Cook County, in partnership with Caremark and the National Association of Counties (NACo), introduced the new Cook County Prescription Drug Discount Card Program. This free card is available to all county residents and can provide significant savings on prescription drugs with no income requirements, no age requirements, and unlimited use for the whole family.

Any county resident without prescription coverage can use this program. Even if you have insurance for prescription drugs, you may still benefit from the discount card, since it may save you money on prescription drugs your existing plan does not cover.

 

Access to Care

The department is the lead affiliate for implementing the Access to Care Program, in conjunciton with the suburban Primary Health Care Council. Access to Care provides subsidized health services to suburban Cook County residents with limited financial resources. Through a partnership with private physicians, care is provided tomroe than 13,000 suburban residnets at private clinics located throughout the suburbs.

Please be advised that as of January 31, 2003, the Cook County Department of Public Health has transferred its Vital Records department to the Cook County Clerks office and will no longer be providng the public with birth and death certificates. 

Please visit the Cook County Clerks website at www.cookcountyclerk.com and click "Vital Records" for instructions and locations to obtain your records.
