The following information is intended for Funeral Directors to assist them in working with the Office of the Medical Examiner.

 

 

Cremation Permits

Cremation permits are fifty dollars ($50.00) each.

 

There are two types of cremation authorizations:

  1. Hand written permit
    Fetal Death Certificates, Hand Signed Death Certificates, and deaths where the disposition is a donation must come to the Cook County Medical Examiner’s Office to obtain a cremation permit.
     

  2. Authorized through IVRS

    In order for a cremation to be authorized through IVRS, the funeral home/crematory must have a Fax Account with the Medical Examiner’s Office. Call 312-997-3025 to inquire about opening an account and requirements for use.

Hours: The Medical Examiner's Office operates 24 hours per day, 7 days per week. The office is open to the general public: 8:30 a.m. to 4:30 p.m., Monday through Friday, except for official Cook County holidays.

 

Identification Hours:  11:30 a.m. to 4:00 p.m., Monday through Friday. 12:00 p.m. to 4:00 p.m., weekends and official Cook County holidays.
