The DUI Memorial Program is offered in Cook County to provide families an opportunity to safely commemorate a loved one who died as a proximate result of a crash caused by an impaired driver under the influence of alcohol, other drug or drugs, intoxicating compound or compounds, or any combination thereof, as well as to raise the public’s awareness of the consequences of driving while impaired under the influence.
Under this program, a DUI Memorial Marker may be installed and maintained by the Cook County Department of Transportation and Highways (Department) for a period of at least two (2) years for fatal crashes which occurred on or after January 1, 1990 along a highway under the jurisdiction and maintenance of the Department. A DUI Memorial Marker consists of a Sign, and Plaque (optional), such as shown above which will normally be installed and maintained by the Department at or near the location of the crash. However, the Department reserves the right to install the DUI Memorial Marker at an alternate location due to restricted right-of-way, property owner complaints, interference with essential traffic control devices, safety concerns, or other restrictions. After the two (2) year period, the Department keeps the purchased Sign, but returns any purchased commemorative Plaques to the applicant.
An immediate relative of a deceased individual of a crash caused by a driver found to be under the influence of alcohol, other drug or drugs, intoxicating compound or compounds, or any combination thereof, may apply for a DUI Memorial Marker through this program by submitting an official application form to the Department. AAIM, MADD or other interested groups may assist the immediate relative in obtaining and filling out the official application form. The Department’s goal is to complete the application review process within thirty (30) business days of having received an official application with proper documentation. If approved, the Department will include a copy of the application to the applicant along with instructions concerning payment of the one-time fee. Payment in the form of a cashier’s check or money order made payable to “Cook County Treasurer” are accepted forms of payment. Payment cannot be made until the application is approved by the Department, and the applicant is notified accordingly. The DUI Memorial Marker shall be installed within thirty (30) days of receipt of payment. The applicant shall be notified once the DUI Memorial Marker has been installed.
